Medications are processed by our liver by an enzyme called cytochrome P450 (CYPs). This enzyme breaks down medication reducing blood levels of them.

Grapefruit and other fruits of the same family have furanocoumarins which prevent P450 from working normally which leads to an increase in blood levels of some medications.

This blood level of medication can increase so much that it can be dangerous. Because of this, grapefruit can increase and worsen some side effects of the drug.

In some studies, apart from P450 inhibition, it has been shown that it also inhibits OATP (organic anion transporting peptide) that as its name says, is a transporter for some drugs. So, other drugs can have decreased effects.
